I am a physician. I had an evaluation at Rush Behavioral Health, Oak Park, Special Assessment Services in April 2004. Perhaps six weeks ago, I requested medical-psychological-counseling records for that evaluation, and was referred to another organization, whose name I forget. Perhaps the name had the word Lutheran in it; apparently this organization has taken over the "Special Assessment Services." I did not save copy/-ies of the forms requesting the records, so the forms I sent were the only copies that I had of the name, address, phone numbers, etc of that organization. ANYWAY, can you help me get back in touch with the medical-psychological-counseling records department for that April 2004 evaluation whoever it now is? Preferably by phone since it has been weeks, and I have already sent the forms requesting copies of my records. Thanks a bunch!
